ABLT is Expanding Support to New York’s Children, Youth and Families
ABLT is pleased to partner with The Collaborative for Children & Families, Inc (CCF), the largest lead Health Home designated to serve Children ages 0-21.
Doing it alone can feel nearly impossible!
ABLT supports and assists families to connect them to all the resources they are entitled to and need so that they can remain healthy, safe in their homes and communities.
Every parent can use a little help but having a child with serious medical, emotional, developmental, or behavioral challenges requires even more assistance and support.
ABLT will help navigate the multiple systems of care to support individuals and their families by offering them help with some of the following:

Obtaining food stamps, Section 8 housing, and other state entitlements.

Advocating for children, youth, and families who need help with their multiple healthcare needs.

Focusing on the strengths of the individual

Helping schedule appointments and arrange transportation.

Connecting family to free community resources.

Communicating with other service providers to break down language barriers.

Supports individual through difficult activities of daily living.

Help transitioning individuals who are returning to live at home.

Offer a holistic approach that includes the whole family.

Bringing multiple service providers to the table to help children and young people with various health and mental health needs thrive and grow.
A Comprehensive Approach to Health
Being healthy is about more than physical health. Children and teens need a host of supports – a healthy diet, a safe home, an appropriate educational setting – to truly be healthy. Our Care Management services, make certain all those needs are met. We help qualifying Medicaid-eligible families & individuals to make sure they receive a whole range of care and services.
Our Care Managers are skilled and trained to communicate and share information in ways that are meaningful to our clients, sometimes acting as translators. Our services also ensure care during transitions from inpatient to other settings and promote ongoing medical and behavioral care to prevent crisis. Ultimately, we focus on securing the long-term success for every child and his/her family.
Who Qualifies for Care Management Services?
Care Management services are available to any Medicaid-eligible individual in New York State with one or more qualifying conditions. Basic qualifications include:

Active Medicaid insurance coverage

Single Qualifying conditions may include:

HIV/AIDS
-
Sickle Cell Disease
-
Complex Trauma
-
Serious Emotional Disturbance
(Serious Emotional Disturbance is often indicated by a child’s inability to learn or develop or maintain relationships with others, while Complex Trauma is common among youth who have been victim of or witness to abuse, neglect, violence in the home or the community, or who have been displaced from their homes or had parent figures removed from their lives)

Individuals must also be at risk of experiencing a significant life event if not enrolled with the Health Home program. Examples of these can include:

Inability to manage medication regimen

Placement in Foster Care

Nursing Home placement
